Below are the forms that need to be completed for every student.  Please print,  fill out, and return them on your scheduled orientation day. 
Copies are also available in the Middle School Main Office.

The following forms are available,  and not every student needs to complete them:
Permission to Medicate (use only if needed)
Fee Wavier Form (only if eligible and available at the main office)
Free and Reduced Lunch Form (available on the first day of school)
